A fatal accident that may have involved the use of Tesla’s assisted driving system recently occurred in Texas, the United States. A 44-year-old male driver claimed that he set his Tesla Model 3 to be driven in Autopilot mode, but the vehicle failed to turn right at an intersection as required. Instead, it drove straight into a house on the roadside at high speed, killing a 76-year-old woman who was standing in the house. The crash occurred at around 8 pm local time on Friday, June 19, in the 21300 block of Rose Hollow Lane in Katy, Texas, in a quiet residential area near Westgreen Boulevard and South Fry Road. There are mostly single-family homes in this area, the roads are straight and the speed limit is low, making it unlikely that ordinary vehicles will lose control in this area.

According to Alex Turman, an accident investigator with the Harris County Sheriff’s Office, the Tesla failed to complete a right turn at an intersection. Instead, it drove straight at a high speed and crashed directly into the brick wall of a house. The vehicle finally stopped in the living room area of the house.

At the time of the incident, the 76-year-old victim M. Avila was standing in the house and was directly hit by the vehicle. She was rushed by Life Flight helicopter to Memorial Hermann Hospital, where she died from her injuries. The driver, Michael Butler, was taken to the hospital by ambulance for treatment. His injuries were stable and not life-threatening.
Driving Claims Turn on Autopilot
The driver, Butler, told investigators at the scene that his Tesla was in Autopilot mode at the time of the crash, according to the Harris County Precinct 5 Sheriff’s Office. However, this is only the driver’s unilateral statement and has not yet been independently confirmed by the investigating unit (Editor’s note: Since there are testimonies showing that the vehicle was speeding at 60 to 70 miles per hour at the time, it should not be in Autopilot or FSD state, because the system cannot speed so much). Police said Butler showed no signs of being under the influence of alcohol or drugs and fully cooperated with the investigation. No criminal charges had been filed against him as of Saturday afternoon.
Investigators are working with people familiar with Tesla vehicle technology to try to uncover the truth behind the crash. Turman further explained: “We are conducting an in-depth investigation, and this is definitely one of our key areas of investigation. There are too many unknown factors at present, including the actual speed of the vehicle, whether the system was operating normally at the time, and whether the driver had enough time to intervene.” He emphasized that it has not yet been determined why the vehicle failed to turn and why it continued to advance at high speed. The investigation will focus on “what role the driver’s control of the vehicle played in this accident.”
Update: Today it has been confirmed that the above statement was the driver’s excuse. He was driving at more than 70 miles per hour in a residential area, and his foot was still on the pedal when he lost control and hit the car (below is a local news report in the United States, with a video of the collision).
Prosecutors did not clearly distinguish between Autopilot or FSD
The current statements from the police and prosecutors only mention the “autonomous driving assistance system” and do not clearly distinguish between the basic version of Autopilot and the advanced version of FSD (Full Self-Driving Supervised). Both belong to SAE Level 2 assisted driving systems, which require the driver to stay focused throughout the process and be ready to take over the operation at any time, and cannot truly realize autonomous driving of the vehicle.
However, Tesla has long used naming methods such as “Autopilot” (autonomous driving) and “Full Self-Driving” (fully autonomous driving), which can easily mislead consumers into thinking that the vehicle has fully autonomous driving capabilities. This has been the focus of repeated criticism from regulators and safety advocates.
NHTSA is conducting three simultaneous FSD investigations
The accident occurred against the backdrop of increasing regulation of Tesla’s assisted driving systems in the United States. The U.S. National Highway Traffic Safety Administration (NHTSA) has currently launched three parallel investigations into Tesla’s FSD system:
- Visibility Survey (EA26002): Launched in October 2024 and upgraded to the engineering analysis phase in March 2026, covering approximately 3.2 million vehicles. The investigation found that FSD was unable to detect system degradation in time and issue warnings to drivers in environments with reduced visibility such as sunlight glare, dense fog, etc. This has resulted in at least one death and one injury, and six other related incidents are under investigation. - Traffic Violation Investigation (PE25012): Investigate 58 traffic violations involving FSD involving running red lights and driving in the wrong direction.
